POSITION SUMMARY:

Responsible for Long Term Care Unit that includes operation of Information & Referral, Protective Services, Caregiver Support Program, OBRA, Level of Care Determination, OPTIONS Program, Care Management and related contract community services.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

  • Reviews all Long-Term Care department programs for compliance as required by PA Department of Aging and in accordance with federal, state, and county policy and procedures as they relate to casework/client services. Maintains and updates procedure manuals for Long Term Care Unit and ensures compliance in implementation of all directives.Plans for the organization of the unit to carry out same.
  • Prepares Work Specifications section of contracts for Long Term Care services and gives input on Selection Criteria; participates in provider selection process as applicable.
  • Establishes and maintains necessary unit controls for ordering services to ensure that program expense and income are kept within Agency budget. Communicates same with Deputy for Administration.Reviews and authorizes invoices for LTC services for payment.
  • Establishes and maintains necessary system of internal/external monitoring to ensure client access to long term care with required prioritization and quality control needed for care planning, care management and community services.
  • Assists in development of Agency’s annual plan and budget through analysis of program data and operations, with Director and Deputy for Administration.
  • Prepares program reports, evaluations and correspondence.Responsible for use of multiple data systems for tracking of client and services.
  • Directly supervises and meets regularly with assigned staff. Oversees all personnel actions of Long Term Care staff and coordinates interviews for vacant positions with recommendations to Director.Responsible for performance evaluation for assigned staff.
  • Assists the Director in exploring possible new business initiatives for the department, and oversees implementation of selected options.
  • Coordinates field placement activities of social work students assigned to service delivery component of Agency.Ensures orientation and assigns to service delivery units.
  • Assists Director in identifying staff development needs.Serves on PA Association of AAA committees as assigned, and attends P4A quarterly meetings as directed.
  • Attends meetings, speaks to groups, and interprets Agency programs to community as Director’s representative.
  • Serves as Acting Director in his/her absence.
  • Other duties may be assigned.

QUALIFICATIONS REQUIRED:

  • Master of Social Work Degree preferred plus five years’ professional experience in human services program either in an administrative capacity or in providing social services to clients, including two years of supervisory experience. License in human/social services field preferred.
  • Knowledge of basic principles and objectives of social service programs, particularly in aging field.
  • Knowledge of principles of public administration.
  • Knowledge of basic fiscal and programmatic principles and practices.
  • Ability to plan, organize and direct the work of others.
  • Ability to gather and analyze information and apply sound judgment in arriving at appropriate conclusions.
  • Ability to establish and maintain effective working relationships with associates, public officials, administrators and the general public.
  • Ability to express ideas orally and in writing.
  • Ability to travel within the county and to regional and state meetings or trainings.
