POSITION SUMMARY:

Based in Doylestown, PA with responsibilities throughout Bucks County, PA. This position will provide HIV Navigation Services to individuals at high-risk for HIV infection, and re-engagement for people living with HIV that are lost to care. This position identifies psychosocial barriers that preclude engagement in care, and facilitates actions to ensure individual’s retention in HIV-related care, medication adherence, and prevention services.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

  • Engaging individuals at high-risk for HIV infection (e.g., repeated STD infections, sexual partner is living with HIV, etc.) and developing a seroconversion prevention plan; emphasis on pre-exposure prophylaxis (PrEP).
  • Engaging lost-to-care individuals to facilitate access and re-engagement in care (i.e., primary medical care [3-4 visits], Medical Case Management, entry into substance abuse and/or mental health treatment, treatment adherence, and bringing others into care). Follow-up with re-engaged individuals to ensure retention in care, and provide additional supports as needed.
  • Performing risk-reduction counseling including barriers (i.e., condoms) and biomedical interventions (e.g., treatment as prevention [TasP], PrEP) for persons in serodiscordant relationships.
  • Assessing people living with HIV for various program eligibility, including determination of Ryan White, DHS Medical Assistance, etc.
  • Providing Health Literacy/Health Education (counseling) to individuals (e.g., education on the HIV service delivery system, how to work with their clinicians, and disease progression and managing life with HIV disease).
  • Identifying barriers along the continuum of care for any given individual, and identify any social supports needed to better facilitate access to prevention and care.
  • Linkage Agreements (MOU/MOA) and referrals, working with key points of entry to create connections between services and funding streams, routine provider meetings, face-to-face contact, and providing referral to additional services to meet immediate needs.
  • Developing a system for monitoring and tracking referrals.
  • Collecting and compiling program data for grant reporting.
  • Performs other related duties and responsibilities that may be assigned

QUALIFICATIONS REQUIRED:

  • Bachelor’s degree in human services, social work, public health, or other related field required; additional counseling experience preferred.
  • 3 years of experience working in the field of HIV is required.
  • Experience working with a variety of populations at high-risk for HIV infection, which may include, men who have sex with men, transgender individuals, incarcerated individuals, and others.

    Other Requirements:

  • Reliable transportation and a valid driver license.
  • Flexible work schedule (position may require working early mornings or evenings).
  • Strong written and verbal communication skills; public speaking experience a plus.
  • Excellent interpersonal skills and ability to work with a wide range of people.
  • Strong organizational skills for handling multiple tasks at once.
  • Ability to maintain records and to prepare a variety of reports.
  • Proficient in Microsoft Office. Can use a computer and smart phone.
