POSITION SUMMARY:

Full time position that reports to the Deputy Director or designee.  Work will include providing medical consultation to AAA staff and nursing assessment and monitoring visits to homebound frail elderly clients of the AAA.  Work will involve both office and field work.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

  • Provides nursing consultation to Area Agency on Aging (AAA) staff to assist in the assessment and care management process (e.g. consultation on questions regarding medications, components of care plans, etc.).
  • Performs Level of Care and related health assessments in consumers’ current location for consumers being served via a variety of AAA programs. Overall management of client remains with AAA Care Manager and all assessments are reviewed and cosigned by AAA Supervisor.
  • Provides home visit and nursing assessment for all clients for whom AAA plans to fund home health aide services.
  • Provides home visit and nursing assessment of medically unstable clients as referred by Care Managers. Assists the Care Manager in the nursing evaluation of selected elder abuse cases.
  • Functions as a liaison to any medical personnel as needed.
  • Participates in meetings, case conferences, and appeals as needed to provide medical/nursing input.
  • Maintains records, data and submits monthly reports as required by AAA.
  • Provides educational information in response to requests from AAA staff and the public.
  • Maintains and supports the patient’s right of security, dignity and confidentiality.
  • Maintains current PA nursing licensure and provides copy to administration.
  • Employees may be required to perform additional work that follows prescribed or well-established procedures that can be learned within a reasonable time by training on the job.More detailed instructions, as to the nature of their duties, will be given to employees at the beginning of work.

QUALIFICATIONS REQUIRED:

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.  The requirements listed below are representative of the knowledge, skill and/or ability required.  Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

  • One (1) year as a Public Health Nurse I with the Bucks County Department of Health; or Bachelor's degree in Nursing from a baccalaureate program accredited by the National League for Nursing; or one (1) year of public health nursing experience and graduation from a state approved school of professional nursing; or three (3) years of recent nursing experience in a healthcare setting where the nursing process of assessment, diagnosis, treatment and evaluation of individuals and/or families was practiced on a regular basis; or any equivalent combination of experience and training.
  • Possession of a current license to practice as a registered nurse as issued by the Pennsylvania State Board of Nurse Examiners or eligibility for such license.
  • Knowledge of the theories and practices of professional nursing as well as professional community health nursing.
  • Knowledge of the purpose and functions of other health care and social organizations in the community.
  • Knowledge of current developments in the field of public health nursing.
  • Skill in the application of professional public health nursing practices.
  • Skill in the application of basic nursing practices and methodology.
  • Skill in directing and instructing non-professional nursing personnel in the application of nursing practices and methodology.
  • Skill in maintaining records and preparing reports, i.e., nursing care plans and individual and family records.
  • Skill in the application of techniques utilized in interviewing, observing, recording and reporting signs and symptoms of the individuals and family's actual or potential health status.
  • Skill in providing guidance to individuals and families for health promotion and maintenance.
  • Ability to make independent judgments in relation to the nursing care of individuals and families.
  • Ability to make independent judgments in resolving a variety of life-threatening health problems.
  • Ability to plan and assess immediate and long-term health goals of individuals/families.
  • Ability to plan and coordinate the activities of the non-professional nursing personnel.
  • Ability to recognize patterns and trends in the health care delivery system.
  • Ability to acquire new knowledge, skills and techniques required to stay current in public health nursing practice.
  • Ability to develop and maintain effective working relationships with other employees.
  • Ability to establish and maintain effective communications with individuals, families and other members of the health care system.
  • Ability to provide guidance to individuals and families in health and maintenance.
  • Ability to give, understand and follow oral and written instructions.
  • Ability to instruct and direct non-professional nursing personnel in the care and treatment of individuals.
  • Ability to identify actual or potential health and safety hazards.
  • Ability to acquire new knowledge, skills and techniques required to stay current in community health nursing practice.
  • Ability to express ideas clearly and concisely both orally and in writing.
  • Ability to recognize the limitations of one's capabilities and to seek help and/or guidance when necessary.
  • Ability to learn the purpose and functions of other health care and social organizations in the community.
  • Ability to secure appropriate transportation in order to travel to distant places to render professional nursing services.
