POSITION SUMMARY:

This is advanced level professional nursing with primary orientation in the promotion and maintenance of the physical and psychosocial health of individuals and families. Performs as a coordinator of specialized county-wide programs of a major magnitude with responsibility for planning, evaluating, modifying and improving continuity of services. Independent judgment is required for various leadership tasks and program responsibilities. This position requires performance in the same variety of settings and has the same direct nursing service responsibilities as the level II nurse. Work assignments are received in the form of oral and written instructions and employees exercise independent judgment regarding the application of professional nursing practices. Work is reviewed by a professional nursing supervisor through observation, reports and conferences for quality and adherence to established standards.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

  • Directs and/or coordinates the provision of nursing services to insure the continuity of care and the attainment of program goals.
  • Plans, prioritizes, coordinates and evaluates the clinic services, home visit services and health education programs.
  • Coordinates the provision of nursing services based on referrals from individuals, community healthcare providers, social service agencies and surveillance data.
  • Reports on program activities for the purpose of evaluating and modifying to enhance provision of service.
  • Coordinates nursing in-service education for assigned programs.
  • Plans and assesses immediate and long term goals for the assigned area of responsibility.
  • Provides direct nursing services in clinics, home visits and the community.
  • Insures that health promotion and maintenance are provided through teaching, guidance and counseling to individuals, families and groups in the community.
  • Orients new employees to the public health program and to specific assignments in the program.
  • Directs the work of professional and non-professional staff engaged in the program activity.
  • Coordinates or participates in epidemiological surveys, health research activities and special projects.
  • Prepares and submits records and reports in compliance with program mandates.
  • Participates in formal staff development and continuing education programs.
  • Employees may be required to perform additional work that follows prescribed or well established procedures.
  • Skill in basic nursing practices and in the application of public health nursing.
  • Skill in directing professional and non-professional staff in program modalities.
  • Skill in maintaining records and preparing reports with electronic systems.
  • Skill in application of techniques used in interviewing, observing and recording.
  • Skill in providing guidance to individuals and families regarding health promotion and disease prevention.
  • Ability to make independent judgment related to nursing care and the resolution of threats to the health of individuals and families.
  • Ability to assess immediate and long-term goals that are program specific.
  • Ability to properly utilize staff to meet program needs.
  • Ability to plan and coordinate activities of professional and non-professional staff.
  • Ability to develop and maintain effective work relationships with staff.
  • Ability to communicate effectively.
  • Ability to recognize actual or potential health/safety hazards.
  • Ability to recognize patterns and trends in disease surveillance.
  • Ability to express ideas concisely both orally and in writing.
  • Ability to understand and follow oral and written instructions.
  • Ability to recognize the limitations of one’s ability and to seek guidance.
  • Ability to acquire new knowledge, skills and techniques required to be current in public health nursing practice and in compliance with the State Board of Nursing’s annual completion of CEU’s.

QUALIFICATIONS REQUIRED:

  • Baccalaureate degree in nursing. (BSN) or BS from other related health care program.
  • Four (4) years of public health nursing experience.
  • Possession of a current license to practice as a professional nurse as issued by the Pa. State Board of Nurse Examiners.
  • Knowledge of the theories and practices of professional nursing as well as public health nursing.
  • Knowledge of research methods including statistical evaluation and advanced programs computer skills
  • Knowledge of the purpose and function of other health care and social organizations in the community.
  • Knowledge of current developments in the field of public health nursing specific to the program area.
